Pathways Community HUB
Our Pathways Community HUB is a pilot program designed to reduce health and social risks for pregnant and postpartum moms. Offered at no cost to Williamson County participants, this community-based program improves health outcomes by connecting families to the resources they need to thrive.
Our Pathways Community Health Workers (CHWs):
- Build trusting, one-on-one relationships by meeting moms where they feel most comfortable—whether at home, the doctor’s office, or a community center. Many moms feel anxious about accessing health care or food benefits, and we help them gain confidence and advocate for their needs. Our CHWs provide services in English and Spanish and can arrange for translators as needed.
- Serve as a link between the healthcare system, patients, and families/caregivers. Many families juggle multiple needs, and navigating pregnancy can feel overwhelming without knowing what resources are available to help. Our team stays updated on local resources and best practices to offer guidance to families and connect them to the right support.
- Connect with other community organizations. Our CHWs actively network at community events and healthcare opportunities, building strong connections with other organizations and sharing information with local providers. This helps families feel more comfortable when accessing services and streamlines referrals, ensuring they receive the right support quickly. We also track outcomes to improve care. This collaborative approach creates a more integrated, accessible system of support for families.
